A 26-year-old cisgender woman with HIV presents for follow-up. She tested positive for HIV about 2 months ago but has not yet started antiretroviral therapy. One week ago, she learned that she is pregnant. Based on her menstrual history, she is at 9 weeks of gestation. Laboratory studies obtained at her initial HIV evaluation visit 3 weeks prior showed a CD4 count of 632 cells/mm³ and an HIV RNA level of 41,880 copies/mL. Results from the HIV genotypic drug resistance test are pending.
